Abstract
Maligni melanom je zloćudni tumor koji se stvara iz promijenjenih melanocita u pigmentiranom području kože ili benignih nevusa. Karakteriziran je abnormalnim staničnim ciklusom, inhibiranom apoptozom, angiogenezom, invazivnošću i metastaziranjem. Melanocit prelazi u melanoma u trenutku mutacije Ras-protoonkogena, najčešće na regijama koje su izložene suncu. Potporanj metastazama stanica melanoma pogoduje specifično vezanje adhezijskih molekula za ligande kao i proteolitički enzimi koji... More olakšavaju prelazak melanomskih stanica u krvni i limfni sustav. Prognostička procjena melanoma bazirana anatomskom, patohistološkom i kliničkom slikom temelji se primjenom triju klasifikacija – Breslowa, Clarkova i TNM-a. U anatomskom se smislu melanoma dijeli u četiri stadija, gdje viši stadiji ukazuju na prisutnost metastatskih procesa. Patohistološka podjela obuhvaća pet osnovnih tipova melanoma: a) lentigo maligna ( melanomain situ) b) lentigo maligna melanom c) površinsko šireći (melanoma superficiale) d) nodularni melanom e) akrolentiginozni melanom U nuklearnomedicinskoj dijagnostici melanoma primjenjuje se procjena sentinel limfnih čvorova s limfoscintigrafijom pomoću radiofarmaka i gama kamere kao i vitalne boje. Ukoliko sentinel limfni čvor, temeljen patohistološkom dijagnozom, ne sadrži tumorske stanice, tada nema metastaza i bolest je u početnom stadiju, čime se primjenjuju manje radikalne metode liječenja, a rezultati liječenja su optimalna, uz očuvanu kvalitetu života bolesnika.
